OCR description
The covered entity (CE), Gruber Dental, reported that two of its servers were encrypted by a ransomware virus that affected the electronic protected health information (ePHI) of 4,624 individuals. The ePHI involved included names and appointment information. The CE notified HHS, affected individuals, and the media. Following the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its ePHI.
