CareFirst Blue Cross Blue Shield

The covered entity (CE), CareFirst Blue Cross Blue Shield, reported that an employee was the victim of an email phishing attack that affected the electronic protected health information (ePHI) of 6,200 individuals. The ePHI involved included names, dates of birth, Social Security numbers, and health insurance information. The CE notified HHS and affected individuals. In response to the breach, the CE implemented additional technical safeguards and retrained its staff on recognizing and responding to fraudulent email communications. OCR obtained assurances that the CE implemented the corrective actions noted.
