OCR description
Tiger Vision, LLC, the covered entity (CE), reported that an unencrypted laptop was stolen from its facility. This breach affected 2,553 individuals. The electronic protected health information (ePHI) involved included names, dates of birth, and diagnoses. In response to this breach incident, the CE provided notice to OCR, affected individuals, and the media. The CE also notified local law enforcement, encrypted its laptops and workstations, and improved physical safeguards to prevent similar incidents.
