OCR description
Billings Clinic, the covered entity (CE), reported that a hacker gained entrance to its computer network via a phishing incident. During February 22-28, 2018, the hacker accessed email containing protected health information (PHI) on Billings’ network, affecting 949 individuals. The CE provided breach notification to HHS, the media, and affected individuals. While the breach was under investigation, CE reported a similar breach to OCR, and OCR has consolidated all issues from this breach into the investigation of the subsequent breach.
