OCR description
The covered entity (CE), Holland Eye Surgery and Laser Center, reported that it was the victim of a hacking attack that affected the electronic protected health information (ePHI) of 42,200 individuals. The ePHI involved included names, addresses, dates of birth, Social Security numbers, and health insurance information. The CE notified HHS, affected individuals, the media, and provided affected individuals with complimentary credit monitoring services. In response to the breach, the CE implemented additional physical and technical safeguards.
