RISE Wisconsin, Inc.

The covered entity (CE), RISE Wisconsin, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 3,731 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE offered free credit monitoring services and implemented additional technical safeguards to better protect its PHI. Staff training on identifying unauthorized access and information security best practices was also increased.
