Massac County Surgery Center dba Orthopaedic Institute Surgery Center

The covered entity (CE), Massac County Surgery Center dba Orthopaedic Institute Surgery Center, reported that an employee was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 2,000 individuals. The ePHI involved included names, dates of birth, Social Security numbers, and treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards and retrained its staff on recognizing and responding to fraudulent email communications. The CE provided credit and identity theft monitoring services to affected individuals. A complimentary toll-free telephone number was established for questions or concerns. OCR obtained assurances that the CE implemented the corrective actions noted.
