OCR description
Healthland Inc. (HL), a business associate (BA), reported that a sub-vendor, InteliChart, introduced a vulnerability to a provider practice portal. This vulnerability enabled patients to view the appointments of other patients. This breach affected approximately 614 individuals. The protected health information (PHI) involved included names and treatment information. HL notified HHS, affected individuals, and the media. Following this breach, HL implemented additional security safeguards to further protect its PHI.
