Med Associates, Inc.

The covered entity (CE), Med Associates, reported that an unauthorized individual gained access to an employee’s workstation and was able to access the protected health information (PHI) of approximately 276,057 individuals. The PHI involved included names, dates of birth, addresses, dates of services, diagnoses, and other treatment and health insurance information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE provided complimentary credit monitoring services to affected individuals and implemented additional administrative and technical safeguards. During its investigation, the CE notified HHS that it was dissolving its business operations; and in response, HHS closed its investigation.
