OCR description
The covered entity (CE), Orlando Orthopaedic Center, reported that protected health information (PHI) was viewable over the Internet due to a software error. This breach affected 19,101 individuals and the PHI involved included names, dates of birth, addresses, phone numbers, health insurance information, Social Security numbers, and clinical data. The CE notified HHS, affected individuals, the media, and established a toll-free number for questions or concerns. Affected individuals were also provided with complimentary credit monitoring services. OCR obtained assurances that the CE implemented the corrective actions noted.
