OCR description
The covered entity (CE), J. A. Stokes, Ltd., reported that it was the victim of a ransomware attack involving the electronic protected health information (ePHI) of approximately 3,200 individuals. The ePHI involved included names, addresses, birthdates, drivers’ license information, claims information, diagnoses, lab results, medications prescribed, and x-rays. The CE notified HHS, affected individuals, and the media. The CE has subsequently ceased operations and is no longer in business.
