Catholic Charities Neighborhood Services, Inc.

The covered entity (CE), Catholic Charities Neighborhood Services, Inc., reported that an employee was the victim of an email phishing attack that affected the electronic protected health information (ePHI) of 565 individuals. The ePHI involved included names, dates of birth, Social Security numbers, health insurance information, diagnoses, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substituted notice. The CE also provided complimentary credit monitoring services to affected individuals. In response to the breach, the CE strengthened its administrative, technical, and security safeguards and retrained its staff on the proper methods of recognizing and responding to fraudulent email communications.
