OCR description
The covered entity (CE), Michigan Medicine, reported that its business associate (BA) University Lithoprinters, Inc., inadvertently mailed protected health information (PHI) to the wrong recipients. This breach affected 3,624 individuals. The PHI involved included names, addresses, phone numbers, and email addresses. The CE notified HHS, affected individuals, and the media. Following this breach, the BA implemented additional administrative and technical safeguards to better protect PHI. OCR obtained assurances that the BA implemented the corrective actions noted above.
