OCR description
Ransomware affected the covered entity’s (CE) computer network which contained the protected health information (PHI) of 30,000 individuals, including names, dates of birth, social security numbers, financial information, insurance information, and clinical information. The CE provided breach notification to HHS, affected individuals, and the media and also notified law enforcement. Following the breach, the CE implemented stronger password management procedures and additional technical safeguards. OCR obtained assurances that the CE implemented additional appropriate privacy and security policies.
