FHN Family Counseling Center

FHN Family Counseling Center, the covered entity (CE), reported that an unencrypted laptop containing the electronic protected health information (ePHI) of 4,458 individuals was stolen from a workforce member’s vehicle. The ePHI involved included names, dates of birth, Social Security numbers, drivers’ license information, medical record numbers, health insurance information, clinical information, and financial information. The CE notified HHS, affected individuals, the media, and provided complimentary credit monitoring services. In its mitigation efforts, the CE implemented technical safeguards to better protect its PHI.
