OCR description
The covered entity (CE), Butler County Board of County Commissioners, reported that an employee inadvertently sent documents containing the protected health information (PHI) of 1,912 individuals to the wrong recipients. The PHI involved included names, dates of birth, and health insurance information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE sanctioned the responsible employee and strengthened its administrative and technical safeguards to better protect its PHI.
