OCR description
The business associate (BA), Choice Rehabilitation, reported that employees were victims of an email phishing scheme that affected the electronic protected health information (ePHI) of 4,309 individuals. The ePHI involved included names, claims information, diagnoses, and other clinical and treatment information. The BA notified HHS, affected individuals, and the media. In its mitigation efforts, the BA implemented additional administrative, technical, and security safeguards to better protect ePHI and retrained its staff on email security.
