Centerstone Insurance and Financial Services (d/b/a BenefitMall)

Centerstone Insurance & Financial Services dba BenefitMall, the business associate (BA) of multiple covered entities, reported that on or around October 11, 2018, electronic protected health information (ePHI), had been exfiltrated as a result of an email phishing attack. The breach affected approximately 111,589 individuals and the ePHI involved included names, addresses, dates of birth, social security numbers, and financial information. All affected individuals were notified as well as the media. As a result of OCR's investigation, the BA implemented new technical safeguards to its e-mail platform, provided targeted re-training to involved workforce members on its policies and procedures related to e-mail security, and provided affected individuals with one year of free credit monitoring services.
