OCR description
Managed Health Services, the covered entity (CE), reported that an employee mailed the protected health information (PHI) of 576 individuals to the wrong recipient. The PHI involved included names, medications, and health insurance information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards to better protect its sensitive data. All staff were retrained.
