OCR description
The covered entity (CE), Integrity House, reported that numerous desktop computers were stolen from its facilities affecting the electronic protected health information (ePHI) of 7,206 individuals. The ePHI involved included names, Social Security numbers, addresses, and dates of birth. The CE notified HHS, affected individuals, and the media. Following the breach, the CE implemented additional physical safeguards to better protected its ePHI.
