OCR description
The covered entity (CE), Sunflower State Health Plan, reported that it inadvertently mailed protected health information (PHI) to the wrong recipients. This breach affected 1,625 individuals. The PHI involved included names and insurance information. The CE notified HHS, affected individuals, and the media. As a result of this breach, the CE implemented administrative safeguards to prevent mailing errors and retrained its staff. OCR obtained assurances that the CE implemented the corrective actions noted above.
