Reproductive Medicine and Infertility Associates, P.A.

The covered entity (CE), Reproductive Medicine and Infertility Associates, reported that it was the victim of a malware attack affecting the protected health information (PHI) of 40,000 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses/conditions, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. The CE also provided complimentary credit monitoring services. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. OCR also provided the CE with technical assistance on the HIPAA Privacy and Security Rules.
