OCR description
The covered entity (CE), AdventHealth Medical Group, reported that it was the victim of a cyber-attack involving the electronic protected health information (ePHI) of 42,161 individuals. The ePHI involved included names, dates of birth, addresses, health insurance information, Social Security numbers, and treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented additional technical and security safeguards to prevent future incidents of this nature.
