OCR description
The covered entity (CE), Rocky Mountain Health Care Services, reported that an employee was the subject of an email phishing attack that affected the protected health information (PHI) of 971 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services and implemented additional administrative and technical safeguards to better protect its PHI.
