OCR description
The covered entity (CE), Henry Ford Health System (HFHS) reported that an employee inadvertently mailed protected health information (PHI) to the wrong recipients. This breach affected 5,578 individuals. The PHI involved included names and medication information. The CE notified HHS, affected individuals, and the media. Following this breach, the CE implemented administrative safeguards to prevent mailing errors and retrained its staff. OCR also provided technical assistance to HFHS about mitigating harm by requesting individuals to destroy or return incorrectly received mailings. OCR obtained assurances that the CE implemented the corrective actions noted above.
