OCR description
The covered entity (CE), Arizona Health Care Cost Containment System, discovered that it mailed the protected health information (PHI) of 3,146 individuals to the wrong recipients. The PHI involved included names and dates of birth. The CE notified HHS, affected individuals, the media, and provided a toll-free telephone number for questions and concerns. It also posted substitute notice on its website. In its mitigation efforts, the CE implemented technical safeguards to better protect its data. OCR obtained assurances that the CE implemented the corrective actions noted.
