OCR description
The covered entity (CE), Frederick Memorial Hospital, reported that an employee was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 760 individuals. The ePHI involved included names, Social Security numbers, and health insurance information. Following the breach, the CE implemented additional administrative and technical safeguards to better protect its ePHI. Employees were retrained on the proper methods of identifying fraudulent email communications.
