OCR description
Sonian, a subcontractor of the covered entity’s (CE) business associate (BA), Apptix, left a computer server port open during a data migration, causing a breach of 277,319 individuals’ protected health information. The types of PHI involved included names, addresses, dates of birth, medical information, and in some instances, social security numbers. The CE provided breach notification to HHS, affected individuals, and the media. It also reported the incident to law enforcement. Following the breach, the CE obtained written assurances from Sonian that it implemented technical safeguards to limit the risk of such incidents in the future, and it ensured that Sonian retrained its staff. OCR reviewed the CE’s BA agreement.
