The Oregon Department of Human Services, the covered entity (CE), reported that numerous workforce members were the victims of an email phishing scheme that affected the electronic protected health information (ePHI) of a minimum of 625,000 individuals. The ePHI involved included names, addresses, birthdates, Social Security numbers, and other identifiers. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards. In response to OCR’s investigation, the CE retrained its staff on the proper methods of recognizing and responding to fraudulent email communications.
