Superior Dental Care, the covered entity (CE), reported that an employee was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 41,768 individuals. The ePHI involved included names, addresses, birthdates, claims data, and financial information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented new administrative and technical safeguards to protect the privacy of its ePHI. OCR provided technical assistance to the CE regarding the HIPAA Privacy and Security Rules.
