OCR description
On March 26, 2019, Desert Valley Dental of Glendale (DVDG) filed a breach report stating that on August 7, 2018, a provider accessed and copied information from DVDG’s database. After OCR initiated its investigation, OCR determined that the entity was not a covered entity since it had gone out of business. The case was closed.
