OCR description
Regional Health Medical Clinic – Flormann Street, the covered entity (CE), reported that its business associate (BA) sent letters containing the protected health information (PHI) of 696 individuals to the wrong recipients. The PHI involved included names and treatment information. The CE notified HHS, affected individuals, and the media.
