Riverplace Counseling Center, Inc.

The covered entity (CE), Riverplace Counseling Center, reported that it experienced a ransomware attack affecting the protected health information (PHI) of 11,639 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, health insurance information, and treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE provided complimentary credit monitoring services to those affected and implemented additional technical safeguards and conducted cybersecurity and phishing awareness training for its employees.
