OCR description
The covered entity (CE), Questcare Medical Services, PLLC, reported that an employee was the victim of an email phishing attack that affected the electronic protected health information (ePHI) of 5,573 individuals. The ePHI involved included names, addresses, dates of birth, medical record numbers, and clinical information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE retrained its employees on email security and implemented additional administrative and technical safeguards.
