Harbor

The covered entity (CE), Harbor, reported that several employees were the victims of an email phishing scheme that affected the electronic protected health information (ePHI) of 29,972 individuals. The ePHI involved included names, addresses, birthdates, Social Security numbers, drivers’ license numbers, health insurance information, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. The CE also provided complimentary credit monitoring services to affected individuals. In its mitigation efforts, the CE implemented new administrative and technical safeguards and retrained its employees on the proper methods of recognizing and responding to fraudulent email communications. OCR provided technical assistance to the CE and obtained assurances that the CE implemented the corrective actions noted.
