OCR description
The covered entity (CE), Madison Parish Hospital Service District, reported that an employee forwarded several emails containing protected health information (PHI) to an unauthorized recipient prior to his/her departure. This breach affected approximately 1,436 individuals. The PHI involved included names, addresses, insurance information, diagnoses/conditions, lab results, and medications prescribed. The CE provided breach notification to HHS, affected individuals, and the media. The CE took legal measures against the former employee which resulted in an injunction against any further use of its PHI.
