Bodybuilding.com LLC, operated by Vitalize, LLC (“Vitalize, LLC”)

Vitalize, LLC, the covered entity (CE), reported that it was the victim of a cyber-attack affecting 3,193 individuals. The protected health information (PHI) involved included names, dates of birth, Social Security numbers, health insurance information treatment information, and claims data. In its mitigation efforts, the CE implemented administrative and technical safeguards to better protect its sensitive data and retrained its staff. OCR obtained assurances that the CE implemented the corrective actions noted.
