OCR description
The covered entity (CE), Area Agency on Aging and Disabilities of Southwest Washington, reported that an employee sent a report containing protected health information (PHI) to his personal email account. This breach affected approximately 7,000 individuals. The PHI involved included names and health insurance information. As a result of the breach, the CE sanctioned the employee and implemented technical safeguards to further protect its PHI.
