OCR description
The covered entity (CE), Health Care Service Corporation, reported that due to a coding error, the protected health information (PHI) of 1,182 individuals was impermissibly disclosed. The PHI involved included names and diagnoses. The CE notified HHS and the affected individuals. In its mitigation efforts, the CE fixed the coding error, revised its policies and procedures, and retrained its employees.
