OCR description
The covered entity (CE), The City of Fort Lauderdale, reported that a document containing the protected health information (PHI) of 757 individuals was posted on its website. The PHI involved included names, Social Security numbers, and health insurance information. The CE notified HHS, affected individuals, the media, and offered complimentary credit monitoring services. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. OCR received assurances that the CE implemented the corrective actions noted.
