Community Health Network, Inc.

The covered entity (CE), Community Health Network, Inc., reported that several employees were the victims of email phishing schemes. The email accounts contained the protected health information (PHI) of 6,600 individuals. The PHI involved included names, dates of birth, Social Security numbers, insurance information, diagnoses/conditions, lab results, and medication information. The CE notified HHS, affected individuals, and the media. As a result of this breach, the CE implemented additional technical safeguards and retrained its staff on the proper methods of identifying fraudulent email communications. OCR obtained assurances that the CE implemented the corrective actions noted.
