University Medical Center Physicians

The covered entity (CE), University Medical Center Physicians, reported that two employees shared protected health information (PHI) using a shared drive without authorization. In addition, one of the employees sent emails containing protected health information (PHI) to a personal email account. These breaches affected 3,300 individuals and the PHI involved included names, addresses, dates of birth, and treatment information. As a result of these breaches, the CE implemented additional technical safeguards to better protect its PHI. The employees responsible for the breach incidents were sanctioned and the CE retrained its workforce members on the need to protect patient privacy and the importance of securing sensitive data. The CE notified HHS, affected individuals, and the media. OCR obtained assurances that the CE implemented the corrective actions noted.
