Midwest Medical Center

Midwest Medical Center (MMC), the covered entity (CE), reported that an employee of its business associate (BA), OS, Inc., (OS), was the victim of an email phishing scheme that affected the protected health information (PHI) of 8,010 individuals. The PHI involved included names, addresses, dates of birth, medical record numbers, and treatment information. MMC notified HHS, the media, and posted substitute notice on its website. MMC and OS notified affected individuals and offered complimentary fraud consultation and theft restoration services. The BA also trained its staff on the proper methods for identifying fraudulent email communications.
