OCR description
The covered entity (CE), Memorial Hermann Health System reported that an employee fraudulently used the financial information of its patients for his/her own personal use. This breach affected approximately 604 individuals. The employee was sanctioned and the CE upgraded its software to more effectively safeguard PHI. The CE notified HHS, affected individuals, and the media. OCR obtained assurances that the CE implemented the corrective actions noted above.
