OCR description
The covered entity (CE), Centura Health, reported that several employees were the victims of an email phishing scheme that affected the electronic protected health information (ePHI) of 7,515 individuals. The ePHI involved included names, dates of birth, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the incident, the CE implemented additional administrative and technical safeguards and retrained its staff. OCR also provided the CE with technical assistance regarding the HIPAA Security Rule.
