OCR description
Takai, Hoover, and HSU, P.A., the covered entity (CE), reported that the Montgomery County Police Department informed them that one of its employees was a person of interest in an ongoing fraud investigation and that she may have used its protected health information (PHI) in fraudulent activities. This breach affected the PHI of 16,000 individuals. The PHI involved included names, addresses, birthdates, and financial information. The CE notified HHS, affected individuals, and the media. The CE implemented additional technical safeguards to better protect its sensitive data.
