OCR description
The covered entity (CE), Worcester Eye Consultants, reported that a medical device containing the protected health information (PHI) of approximately 2,634 individuals was lost in transit. The device was shipped by its business associate (BA), Carl Zeiss Meditec, Inc. The PHI involved included names, dates of birth, clinical information, and treatment information. The CE notified OCR, affected individuals, and the media. During the investigation, OCR provided technical assistance regarding the execution of business associate agreements.
