OCR description
Kingman Healthcare Inc., the covered entity (CE), reported that the configuration of its website made it possible to view protected health information (PHI) over the Internet. This breach affected approximately 1,100 individuals and the PHI involved included names, dates of birth, diagnoses/conditions, and clinical information. The CE notified HHS, affected individuals, the media, and provided substitute notice. Following OCR’s investigation, the CE implemented a number of technical safeguards to prevent a similar breach from happening in the future.
