OCR description
The covered entity (CE), Sunshine State Health Plan, reported that workforce members of its business associate (BA), Consumer Direct, were inadvertently given administrative access rights to its portal that contained the protected health information (PHI) of 2,544 individuals. The PHI involved included names, home addresses, and email addresses. In its mitigation efforts, the BA implemented additional administrative and technical safeguards and retrained it staff. The CE notified HHS, affected individuals, and the media. OCR obtained assurances that the BA implemented the corrective actions noted.
