OCR description
Inform Diagnostics, Inc., the covered entity (CE), reported that its business associate (BA) was the victim of a cyber-attack involving the protected health information (PHI) of 171,575 individuals. The PHI involved included names, addresses, dates of birth, and financial data. The CE notified HHS, affected individuals, and the media.
